In Grade 11

• Register with the Eligibility Center at http://www.eligibilitycenter.org/

• Make sure you are on track to meet the core course requirements (NCAA Core Classes) for the Eligibility Center.

• At the end of junior year, send a copy of your transcript to the Eligibility Center. If you have attended any other high schools, make sure a copy of your transcript from each school you have attended is sent to the Eligibility Center.

• Request test scores from either the ACT or SAT be sent directly to the Eligibility Center (the NCAA code is 9999).

• Begin your amateurism questionnaire. Tips on how to fill out the questionnaire are listed below.

In Grade 12

• Complete the amateurism questionnaire and sign the final authorization signature online on or after April 1 if you are expecting to enroll in college in the fall semester.

• If you did not submit your ACT or SAT score to the Eligibility Center during your junior year, request that it is sent this year (the NCAA code is 9999).

• After you have completed your finals in June, send a final transcript with proof of graduation to the Eligibility Center.
